Privacy notice

What this site collects, how your records are protected, and how to ask for a copy or a correction.

This notice covers the information we collect through this website and how it relates to your protected health information as a patient.

What this website collects

  • What you type into the consultation request: name, phone, email, preferred office and time
  • Nothing clinical. We ask you not to send medical details through the form
  • No advertising cookies. If you allow analytics cookies in the cookie banner, Google Analytics counts visits for WebRen, which built this concept site; change your choice at any time with Cookie settings in the footer

Your health information

Once you become a patient, your records are protected health information under HIPAA. We use and disclose it to treat you, to bill your insurance, and to run the practice. We do not sell it, and we do not use it for marketing without your written authorisation.

Photographs

We take clinical photographs at most visits, because orthodontics is a before-and-after profession. They stay in your record unless you give separate written consent for us to publish them, and you can withdraw that consent at any time.

Your rights

  • A copy of your records, usually within 30 days
  • A correction if something is wrong
  • A list of disclosures we have made
  • A complaint to us or to the U.S. Department of Health and Human Services, with no retaliation
